School Attendance Forms
If your child is sick and will not be in school, please complete Report Absence form by 10:00 a.m.
If your dismissal plan for the day needs to be changed, please fill out this form by 3:00
If your child needs to leave early (for an appointment), please fill out this form.
School Medication Form
If your child requires medication to be dispensed at school, please print the authorization form below which requires both parent and doctor signatures. Bring form to the school office with the medication in the original container clearly marked with child’s name.

Parent Volunteer Forms
Volunteer Information
Anyone wishing to volunteer in ANY capacity must complete Virtus training (Protecting All of God’s Children) as required by the Diocese of Lansing. Select the link below, proceed with training and contact the school office to advise of completion.
Additional Requirement: All volunteers (in any capacity) must submit a Background Check form (Michigan Waiver Agreement for Schools). Select below, print, fill out and submit directly to St. Patrick School.
